Insurance and vendor documents

To request a current certificate of insurance, send your property address, certificate-holder legal name and address, and your vendor insurance requirements to carlos@jlconstructiongroupllc.com. Include any requested additional-insured wording for review. Coverage, limits and endorsements must be confirmed against the issued documents.

Request a W-9 and any required vendor forms directly. Documents containing tax identifiers are shared privately during onboarding.

Before service begins

Provide each property address, billing entity and authorized decision-maker.
Identify parking areas, sidewalks, entrances, loading areas and any excluded areas.
Share operating hours, access restrictions, priority routes and snow-storage locations.
Confirm the written scope, service triggers, pricing, payment terms and any extra-work approval process.
Provide primary and storm contacts, reporting requirements and your vendor-document checklist.
Confirm the service agreement and route availability before service is scheduled.
